    I give permission for Bible Camp leaders to administer basic first aid to my child as needed. In the event of a medical emergency requiring professional treatment, and if I cannot be reached immediately, I authorize staff to seek emergency medical care, including transport to the nearest hospital or clinic.

    5. Activity Consent

    Score the Goal Bible Camp includes Bible lessons, snack time, arts & crafts, and active indoor/outdoor games (including soccer-based games). As with any group or physical activity, there is a small risk of minor injury.


    6. Photography & Video Consent

    Our team may take photos or video for the purpose of sharing highlights with families, church bulletins, and social media (Facebook, Instagram) to promote future events. This choice will not affect your child’s participation either way.

    7. Behavior & Group Guidelines

    I understand leaders expect all children to treat others with kindness and respect. In rare cases of serious or unsafe behavior, my child may be asked to sit out or be picked up early.

    I confirm the information above is accurate. I release the organizers, volunteers, and host location from liability for accidents or injuries not caused by gross negligence or intentional misconduct, to the fullest extent permitted by law. I understand my data will be used only to run this camp and will be kept confidential.

      Dopuštam voditeljima Biblijskog kampa da mom djetetu po potrebi pruže osnovnu prvu pomoć. U slučaju medicinske hitnosti koja zahtijeva stručno liječenje, a ako me nije moguće odmah kontaktirati, ovlašćujem osoblje da zatraži hitnu medicinsku pomoć, uključujući prijevoz do najbliže bolnice ili klinike.

      5. Suglasnost za aktivnosti

      Biblijski kamp “Zabij gol” uključuje biblijske pouke, vrijeme za užinu, kreativne radionice te aktivne igre u zatvorenom/na otvorenom (uključujući igre vezane uz nogomet). Kao i kod svake grupne ili tjelesne aktivnosti, postoji mali rizik od manjih ozljeda.


      6. Suglasnost za fotografiranje i snimanje

      Naš tim može fotografirati ili snimati djecu radi dijeljenja s obiteljima, crkvenih biltena i društvenih mreža (Facebook, Instagram) za promociju budućih događanja. Vaš izbor neće utjecati na sudjelovanje vašeg djeteta.

      7. Pravila ponašanja i skupine

      Razumijem da voditelji očekuju da sva djeca postupaju prema drugima s ljubaznošću i poštovanjem. U rijetkim slučajevima ozbiljnog ili nesigurnog ponašanja, moje dijete može biti zamoljeno da ne sudjeluje ili bude ranije pokupljeno.
